Workers' Compensation Records

California Adjudicated claims only; provides only claim number and date of incident
Colorado No requirements for claims 1985 to July of 1990. To receive reports from July 1990 to present
a notarized release is required and there is a 7 day wait.
Idaho No requirements for claims contested by employer or employee, updated monthly.
7 days and a release is required for all first reports of injury.
Kansas Special State Form
Missouri Notarized Release is Required on Special State Form
Nevada Notarized Release is Required
South Dakota Original Release is Required (must specifically state workers' compensation records
may be released.
Utah No Requirements for adjudicated claims back 7 years. For all reports of injury requires
a notarized release and takes 7 to 14 days.
Virginia Notarized Release is Required
Wyoming Original Release Required (mailed)
